Our case managers work with participants to identify needs, reduce barriers, and access community resources. After conducting a comprehensive assessment on the participant’s strengths and limitations and the resources available to them, the case manager and the participant work together to determine how the chief concerns identified during the assessment relate to their available resources. An individualized plan is then created collaboratively to identify goals, strategies and resources. The case manager continues to work with the participant, checking in frequently and restructuring the plan as needed.

Case management services are voluntary.
